The Texas Education Code requires
that all public institutions of higher education notify all new students
about bacterial meningitis. It also requires that the institution obtain
confirmation that the student received this information. Please read “Important
Information about Bacterial Meningitis.” Complete the “Notification
of Bacterial Meningitis Information” form online, print it and sign
it. Fax, mail, or deliver the form to
the ACC Records Office. Phone number and address are on the form. New
students may not register until this form is received.
